Application Check List • Youth Peer Support Application Form • Exposure Hours Documentation Form To complete the Youth Peer Support Certification process, you must complete 40 hours of exposure hours at a community-based agency (this can include schools, community centers, or behavioral health agencies). Use the Exposure Documentation Form to verify current exposure hours or an ongoing commitment to complete the 40 hours. • YPS Application Letter of Support A letter of support can be provided by a mental health professional (school counselor/therapist), a supportive adult (must be a professional - not a friend or family member) who is familiar with your story and lived experience, and/or a representative of a community organization that you are involved with. After your application has been received, the review committee will contact you to complete a brief eligibility interview to confirm your readiness and interest. Once your application is accepted, you will need to complete the following steps in order to complete certification as a Youth Peer Support Specialist: Attend 40 hour Youth Peer Support Certification Training o Complete 40 Exposure Hours o Prepare a YPS Certification Portfolio: o Certification Training Certificate o Letter of Support for YPS Certification o Exposure Site Hours Verification o Certification Interview Rubric
