This form is to request access to the training and certification site for the CSE-IT. New cohorts start once a month. If your request is received after the 15th of the month prior you will be added to the next month’s list. Once you have completed certification you will access to the tool via the West Coast Children’s Clinic Page

Agency MOU must be in place before you can start training. Please check with your supervisor or CYFD point of contact if you are not sure. An MOU covering all divisions of CYFD is in place.
