EMDR Specialty Provider Rate Application
Welcome!
The NM EBP Qualification for the EMDR Specialty Rate Are:
- Demonstrated completion of an EMDRIA-approved EMDR Basic Training OR EMDRIA Certified Therapist
- New Mexico Professional Mental Health License
- Be a New Mexico Medicaid Provider
If you meet these standards, complete the application and provide all requested information and supporting documentation. Applications will be reviewed and qualifications will be verified by the Center of Innovation team. Recommendations for approval will be made to the State. Once approved, a letter will be sent via email from the Center of Innovation on behalf of the State with further instructions.
Please note: Incomplete applications will not be processed.
Please make sure your NM Medicaid and NPPES NPI Registry accounts are current and accurate. Your Primary Practice Address must match what you have entered on your NM Medicaid Provider Specialty Rate Application. Incomplete or inaccurate information can delay your approval or termination of your application.
